Job Details

Assist in providing client and foster parent support/crisis management - Facilitation of case-related activities on behalf of the agency - Prepare and process case service forms - Supportive counseling/referrals to needed services - Stay up-to-date on all required training: YSIS and ARC Model Training - Provide in-home services and support to youth and families - Participate in the treatment and follow service plans for youth in care - Maintain proper documentation for all services delivered - Respond and provide on-call crisis support - Attend meetings surrounding youth in placement, including Multi-Disciplinary Team (MDT) meetings, Individualized Educational Planning (IEP) meetings, and on-going collaboration meetings with referral sources and community providers - Work in collaboration with the education system - Complete court summaries and attend court hearings as needed - Collaborate with all systems and individuals serving youth, including legal guardian, juvenile probation offi...Family, Aide, Healthcare, Support, Youth
